How Can Intentions Be Combined During a Single Prayer?

Question
I read on SeekerGuidance that it is permissible to combine the intentions of a makeup prayer with that of non-confirmed sunna prayers.
My question is, how should this multi-intention prayer be performed? A makeup prayer would not require the recitation of an additional sura in the 3rd and 4th rakat, but a non-confirmed sunnah (recommended/mandub) (ex: 4 rakat after isha or 2+4 of salah al awwabin) will require that. Can you provide some clarity on this? For example, which non-confirmed prayers can be combined with fajr or asr/dhuhr/isha? How should the prayer be structured in format? Also, is salah al-duha a confirmed or non-confirmed prayer?

The upshot is that it is not always possible.
The Duha prayer is recommended. Thus it is not an emphasized Sunna. [Shurunbulali, Nur al-Idah]
