If someone had piercings, would pouring water over the openings suffice, or must he insert something wet into the holes for the ghusl to be valid?

One just has to ensure water reaches during a major ritual impurity, however that may be.

One of three actions is obligatory in a purificatory bath (ghusl), one is to wash whatever is possible, without undue difficulty or hardship, of the entire body once. [Shurunbulali, Maraqi al-Sa’adat]
