When Is Ghusl Necessary During Persistent Arousal Symptoms?

Question
I am experiencing symptoms similar to PGAD, where I feel constant physical arousal without any desire or intention. It is involuntary and has severely impacted my ability to eat, sleep, and function normally. I am trying to seek medical help, but have not been successful yet.
This condition is also overwhelming me religiously. I know ghusl is required after orgasm and wudu after arousal, but I cannot tell when (or if) an orgasm occurs, and I cannot control the arousal itself. Do I have to perform ghusl or renew wudu continually in this state? I am deeply distressed and in need of guidance.

A ritual bath (ghusl) will only become necessary if an orgasm follows sexual fluid (mani). If there is no fluid, a mere feeling of sexual arousal will not necessitate a ritual bath.
If preseminal or arousal discharge (madhi) is released, then ghusl would not be necessary, only wudu.
Given the frequency you mentioned, we understand it can be challenging to maintain wudu for prayer. Our suggestion would be to try using a piece of cotton as an inter-labial pad (kursuf). This would block the exit of discharge if it does, and help you throughout the day.

Obligatory (fard) is when the emission of sperm/sexual fluid (mani) leaves its usual place inside the body with or without pleasure, for any reason, such as a wet dream or by looking at someone or something. [‘Ala’uddin Ibn ’Abidin, al-Hadiyya al-‘Ala’iyya; Tahtawi, Hashiya al-Tahtawi ‘ala Maraqi al-Falah]
I want to advise you to seek medical care, also fast often, maybe Mondays and Thursdays, also recite the following verse frequently:
“قُلْنَا يَٰنَارُ كُونِى بَرْدًا وَسَلَٰمًا”
“We ordered, ‘O fire! Be cool and safe (…).’” [Quran, 21:69]
